Not just a "lack of planning": some places/people, it's just a completely crappy method of planning. (Plenty of it, just not done right.)

A nice idea, but, sadly, some of us are always the 'emergency option' (i.e. the fixit people).
I am reminded of our recent migration to Office 365 in the cloud.
